Public facing digital services

2024/25

Increase use of digital correspondence; improve patient access to services through the use of digital and NHS App engagement.

2024/25

Baseline data from multiple ICS partners available in a standardised format in the new Notts Care Record.

New operational tool for Integrated Hospital Discharge data.

Business Change to migrate existing shared care record users towards the Notts Care Record and engage new users with the record.

2025/26

Single health and care record available to all staff with all key organisations onboarded.

Enable the sharing of Notts Care Record with neighbouring partners and enable access from our users to other shared care records.

2026/27

Notts Care Record now in a ‘business as usual’ state with ongoing developments to enrich functionality.

All stakeholder partners across the Notts ICS onboarded on to the Notts Care Record.

Provider Digitisation

Enable the visibility of capacity across all care settings and the ability to schedule the move of patients quickly.

Roll-out of electronic prescribing and drug administration systems.

Ensure approach for transformation is user-centric.

2025/26

Enable all staff to work in any location as appropriate.

Rationalise and modernise diagnostic services and reporting.

Utilise digital to enable integrated care across residential care settings (Enhanced Health in Care Homes framework).

SupportING intelligent decision-making

2024/25

Provide the infrastructure to enable data to be used intelligently for strategic decision-making.

Agreed approach across the East Midlands to develop a Secure Data Environment (SDE) for Research and Development as part of a network of regional sub-national SDEs.

2025/26

Ensure Population Health Management (PHM) approach is supported with appropriate resources including better analytics, tools and techniques, which will support all initiatives.

Secure Data Environment for Research is available to be used for the first tranche of research projects and is contributing to the knowledge base of local clinicians.

2026/27

Embed a systematic approach to developing and monitoring system outcomes, and proactively finding and enabling new interventions.

Ongoing development and sustainable operation of the Secure Data Environment for researchers is obtained via a mixture of funding and national support.

2027/28

Augment Artificial Intelligence and human skills in designing care services.

Secure Data Environment for researchers is fully-funded and a business as usual service.
