The Notts Care Record is a unified platform, granting clinical and care staff access to the most current health and care information about the individuals within their care.
The platform brings together health and social care information from partner IT systems into a singular health and care record. It means that vital information is readily available on an easy-to-navigate dashboard, regardless of the organisation it came from.
By joining forces and utilising information from across our integrated care system, our health and care teams are working together to improve care delivery. This saves time, makes it easier for frontline professionals to find the information they need, and provides better, more holistic person-centred care.

HOW IT WORKS
The Notts Care Record offers an easily accessible insight into an individual’s health and social care journey. This helps to ensure that any recent encounters, changes and potential warnings can be picked up. It also improves decision-making and care coordination, making interactions more seamless.
This data is provided from a range of health and care settings including:
Mental Health Services
The Notts Care Record is being made available for authorised staff in NHS Trusts, Community Health Care, Social Care, and GP Practices. Additionally, our Ambulance Service will also benefit from this data through their own Unified Care Record.
